Those little silicone spacers are everywhere. A podiatrist explains what toe separators can really do for a bunion — and what they can't.
Walk down any pharmacy foot aisle — or scroll for ten seconds on social media — and you'll run into toe separators: little silicone wedges and stretchy spacers promising straighter toes and happier bunions. Patients ask us about them nearly every week, usually with the same question: do these things actually work?
The short answer: they're genuinely useful for comfort, and they are not a cure. The longer answer is more interesting, because the research on toe separators is better than you might expect — it just doesn't say what the packaging implies. One note before we dig in: this article is general education, not a diagnosis. If your foot hurts, the smartest first purchase is an exam, not a gadget.
What a Toe Separator Actually Is
A toe separator (you'll also see "toe spacer" or "toe spreader") is a soft piece of silicone or gel that sits between your toes and holds them apart. The simplest version is a single wedge between the big toe and its neighbor — the classic bunion spacer. Full-foot spreaders separate all the toes at once and are popular in yoga and foot-strengthening circles. Some designs pair the spacer with a fabric sleeve that loops over a toe to keep it from sliding around, and there are nighttime versions that hold the big toe gently outward while you rest. Pharmacy versions are one-size-fits-most; a podiatrist can also mold a custom silicone spacer to your exact anatomy — the type used in the strongest study we'll get to below.
What the Research Says About Bunions
Bunions are where most of the science lives, and the findings are surprisingly consistent.
Pain relief is the best-supported benefit. A 2022 systematic review in Arthritis Care & Research pooled the trial evidence on non-surgical bunion care and concluded that improvement in pain is more likely than any change in the toe's actual angle. That matches what we see in the office: people often feel better in a spacer well before anything measurable changes on an X-ray.
The separator seems to be the ingredient that matters. A 2021 meta-analysis in BMJ Open compared different bunion orthoses and found that designs with a toe-separating element did the most to improve the big-toe angle — the spacer, not the strap or the shell, appeared to be doing the work.
Small alignment changes are possible — with serious commitment. In a 2018 randomized trial in Prosthetics and Orthotics International, people with moderate bunions wore a custom-molded silicone spacer for six hours a night, every night, for a full year. Their bunion angle improved by about three degrees, while the group without a spacer drifted slightly worse. Three degrees won't transform a foot, but slowing — even slightly reversing — the drift is real. Notably, the benefit depended on also wearing sensible, roomy shoes.
Spacers do better as part of a program. A 2018 randomized trial in the Journal of the American Podiatric Medical Association combined a toe separator with gentle joint mobilization and foot-strengthening exercises; the package improved pain, function, and alignment measures, and the gains held a year later. A 2021 network analysis in the International Journal of Environmental Research and Public Health pointed the same direction, ranking exercise plus a separator among the most promising conservative combinations.
And the ceiling is clear. The landmark comparison, a 2001 randomized trial in JAMA, followed patients with painful bunions treated with surgery, an orthosis, or watchful waiting. The orthosis group felt better in the short term — but surgery was far more effective for the deformity itself. Devices manage the problem; they don't remove it.
The Part Nobody Prints on the Package
An adult bunion is a bone that has shifted, not a toe that's merely squeezed. The American College of Foot and Ankle Surgeons, the American Orthopaedic Foot & Ankle Society, and the American Academy of Orthopaedic Surgeons all say the same thing in their patient guidance: pads, spacers, and splints can ease pain and may help slow progression, but they do not reverse the deformity. A separator holds soft tissue in a better position while it's on; take it off, and the toe returns to its habit. That's not a reason to skip them — comfort and slower progression are worth having. It's a reason to buy them with clear eyes.
Beyond Bunions: Where Separators Earn Their Keep
Soft corns between the toes. These tender spots form where two toe bones press the skin between them. A spacer separates the culprits and takes the pressure off — a 2014 study in the Journal of the American Podiatric Medical Association measured meaningfully lower pressure inside the shoe with interdigital silicone devices. Overlapping and crowded toes. A spacer can hold a drifting toe in a friendlier position and stop the skin-on-skin rubbing that leads to irritation.
Hammertoes. A separator can cushion and reduce friction, but it will not straighten a toe whose joint has stiffened into a bend.
After bunion surgery. A small spacer between the first and second toes is a routine part of recovery care, helping maintain the corrected position while soft tissue heals — one place where the humble spacer and the operating room work on the same team.

Diabetes, neuropathy, or poor circulation. When feet have reduced feeling or blood flow, a device that rubs in the wrong spot can quietly break down skin. Give them room. A spacer crammed into a narrow shoe adds pressure instead of relieving it. Pair it with a wide toe box, or wear it barefoot at home. Roomy footwear was part of the recipe in the trial that actually moved the bunion angle.
Add exercises. Toe-spreading and arch-strengthening work pairs naturally with a spacer — and the research hints the exercise may be doing much of the heavy lifting. Custom orthotics can round out the plan by addressing the mechanics that drive the bunion in the first place.
Night use is fine. Nighttime wear was exactly the protocol in the strongest bunion trial. Choose a soft design that doesn't pinch, and take it off if anything tingles or blanches.
When It's Time to Look Past the Pharmacy Aisle
