When Should You Actually See a Podiatrist?
Not every foot ache needs a doctor. But some do — and waiting can make things worse. Here's how to tell the difference.
There's a natural overlap between orthopedic surgery and podiatry With the foot and ankle. Both fields treat musculoskeletal conditions in the lower extremity. The difference is focus: a podiatrist's entire career is dedicated to the foot, which means the depth of experience with forefoot deformity, biomechanics, and nail and skin pathology is simply different from that of an orthopedist who covers the entire body.
The best outcomes often come when these two specialties work in complement rather than in competition.
Where Podiatry Takes the Lead
Forefoot deformity surgery — bunion correction, hammertoe repair, and metatarsal osteotomies — is a core part of podiatric training and practice. At our office, we perform these using minimally invasive techniques with smaller incisions, local anesthesia, and same-day walking, which often appeals to patients who are apprehensive about traditional open surgery.
Tendon disorders below the ankle — posterior tibial tendon dysfunction, peroneal tendinopathy, Achilles tendinopathy — benefit from a specialist who manages these conditions daily. Conservative options include bracing, custom orthotics, and physical therapy coordination, with surgical repair available when needed.
Biomechanics and gait issues — cavus foot, severe flatfoot, limb-length discrepancy — are another area where podiatric evaluation adds value. The structural and functional assessment of the foot is foundational to podiatric training.
When Patients Benefit from Co-Management
Complex ankle fractures, polytrauma cases, and conditions requiring hardware from above and below the ankle are natural co-management opportunities. The orthopedist handles the ankle reconstruction; the podiatrist manages the midfoot and forefoot mechanics, custom orthotics, and long-term biomechanical rehabilitation.
